Registration fee includes access to all sessions (21-25, Nov.) of JCMI and APAMI, a presentation opportunity, CoMHI Programs and Abstract Books, Journals of JCMI and APAMI on CD-ROM, access to IMIA-WG4 Workshop, Conference Handbook and daily morning / afternoon teas.
One-Session Admission Ticket is available only on the venue.

Application is completed. Registraion ID will be issued. Auto-confirmation e-mail will be sent to the registered e-mail address.

* Cancellation and/or changes of your application should be notified in writing (by e-mail or fax) to the registration desk below.

PAYMENT

Credit cards (American Express, Diners Club, JCB, MasterCard and Visa) will be accepted for the early bird registration. Kinki Nippon Tourit Co. Ltd., the official travel agent of CoMHI 2009, wholly handles the conference payment process, so please notice that the name of creditor in your credit card statement appeared will be “Kinki Nippon Tourist Co., Ltd.“, but not “CoMHI“.
An inquiry concerning the payment shall be written (by e-mail or fax) to the registration desk as below.

CANCELLATION AND REGISTRATION FEE REFUND POLICY

Cancellation and/or changes of your application should be notified in writing (by e-mail or fax) to the registration desk as below.
In case of cancellation of registration, your registration fee will be refunded after deducting credit cards charge/commission and the following cancellation fee.
If cancellation notification is received....

On or before Oct. 30, 2009;

JPY5,000-

On/after Oct. 31, 2009;

No refund
